    Step 1

    Mangled Extremity Assessment

Patient presents with severe extremity trauma. Begin systematic evaluation.

    Clinical Notes

    WTA Critical Decisions Evidence-Based Insights:

    • ABC Priority: Avoid Doppler assessment during primary survey - focus on life threats first
    • Ischemia Time: Warm ischemia >6 hours significantly reduces salvage success
    • Scoring Systems: MESS, PSI, NISSSA, LSI, HFS have NOT proven valid in large prospective studies
    • Risk Factors: Age >50, hypotension <90mmHg, Gustilo III B/C, circumferential tissue loss
    • Vascular Assessment: ABI <0.9 indicates need for imaging; CTA preferred over angiography
    • Family Involvement: Include family in decision-making; consider operative photos for documentation
    • Functional Hierarchy: Below-knee > above-knee > hip disarticulation for amputation levels
