Work through the traumatic rectal injury pathway, from AAST grading and diagnostic evaluation through intraperitoneal and extraperitoneal injury management and diversion decisions.

A: Trauma with Potential for Rectal Injury
Patient presents with trauma mechanism suggesting potential rectal injury. Begin initial assessment following ATLS principles.
Clinical Note
Maintain high index of suspicion based on mechanism: pelvic fractures, impalement, straddle injuries, penetrating perineal/gluteal trauma, and sexual assault. Digital rectal examination (DRE) should be performed to assess for blood, tone, and palpable injuries.
